Abstract
Diabetes is among the leading causes of morbidity and mortality in the United States; therefore, effective Diabetes Self-Management Education (DSME) is vital in improving patient outcomes. A needs assessment of the Practice site identified gaps in provider confidence and frequency and consistency of delivering DSME that correlated with poor patient adherence and readmissions. The purpose of this quality improvement (QI) project was to implement a structured, evidence-based DSME training program to improve healthcare provider self-efficacy to provide DSME. The project utilized Bandura's Social Cognitive Theory and the ADKAR change model and employed a quasi-experimental, one-group pretest–posttest design. Ten providers (six Registered Nurses and four physicians) participated in a seven-week intervention using asynchronous online training modules. Provider self-efficacy was measured with the validated C-Scale pre and post training. Results demonstrated a statistically significant increase in provider self-efficacy scale scores (pretest M = 14.9, SD = 2.33; posttest M = 19.8, SD = 2.30; Wilcoxon signed-rank test V=55, p = .001, r = 0.89) that demonstrated increased provider confidence providing DSME. Limitations included small sample size and a single site that may impact generalizability; however, this project illustrates that structured DSME training is a way to enhance provider self-efficacy which could ultimately affect improved patient adherence, improved complications, and decreased rates of readmissions. This work adds to ongoing discussion of the need for DSME, not only in the clinical setting, but also as a component of the healthcare provider's continuing professional development, to ensure longevity in provider performance and improved patient outcomes.
