Abstract

Chronic obstructive pulmonary disease (COPD) is one of the most common chronic illnesses encountered in primary health care, with more than 16 million identified cases and many more undiagnosed. The condition is frequently not diagnosed until later stages due to its insidious onset. Once diagnosed, the disease is often inadequately treated. The purpose of this quality improvement project was to implement an evidence-based practice tool. The project team selected the COPD Assessment Tool [CAT], which ensured Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines. The device provided screening for individuals "at-risk" or with a known diagnosis of COPD. The intervention focused on provider education regarding the procedures in a rural family practice clinic in Northeast Arkansas. Methods for this project included a retrospective chart review pre/post-implementation and education. Medical record review established the need for provider education. The findings set the percentage of active patients within the practice, with a history of smoking and the rate of those with a COPD diagnosis, as a baseline. Primary care provider education convened on the CAT and current COPD management recommendations per GOLD guidelines. Post-implementation chart reviews were completed ____ weeks after provider education. Pre and post-implementation chart review results and data analysis are pending as this is an ongoing project. The project team anticipated results would support primary care provider education as an intervention for earlier recognition and treatment of COPD.
