Abstract
Cultural competence remains essential in primary care settings serving diverse populations, yet staff at a small Southern Texas primary care clinic reported no previous formal cultural competency training in workplace orientation or school. The purpose of this Doctor of Nursing Practice quality improvement project was to evaluate whether a brief, evidence-based cultural competency intervention improved clinician cultural competency scores. Madeleine Leininger’s Culture Care Theory provided the theoretical framework, Lewin’s Change Theory guided the change process, and the Plan-Do-Study-Act cycle directed implementation and evaluation. The problem addressed was the lack of structured training to support culturally responsive communication, patient trust, and improve awareness of barriers in equitable care. A quasi-experimental pre-test/post-test design was used with five clinical staff members, including two nurses, two medical assistants, and one DNP-prepared advanced practice registered nurse. Participants completed the South Texas Cultural Competency self-assessment immediately before and after a 40-minute educational presentation focused on cultural humility, stigma reduction, communication strategies, and culturally responsive care. Mean composite scores increased from 87 before the intervention to 100.4 after the intervention, with a mean gain of 13.4 points, or 15.4%. Four of five participants improved, with the greatest gains noted in knowledge and skills. A paired-samples t test approached but did not reach statistical significance, t(4) = 2.52, p =.066. Findings suggest that brief, low-cost cultural competency training may improve staff culture competency and support more patient-centered, equitable primary care.
